How to participate?

First, you need to be an active Ortho patient or Past Ortho Patient at any of our 6 clinics.

Complete the form below with all your basic information and social media profiles. After submitting your information, you will receive a link to upload your video story. In your video story you need to tell us why you should win this Prom Giveaway. Your video should be ideally between 30 to 60 seconds. Max 2 minutes. 

You need to submit your video story by March 31st or earlier!
